What is Macular Degeneration?
Macular degeneration, also known as age-related macular degeneration (AMD), is a progressive eye condition that affects the central portion of the retina, called the macula. The macula is responsible for sharp, central vision needed for activities such as reading, driving, and recognizing faces. AMD is the leading cause of vision loss in adults over the age of 50.
Types of Macular Degeneration
There are two main types of AMD:
- Dry (Non-neovascular) AMD:
The most common form, accounting for about 80-90% of cases. It is characterized by gradual thinning of the macula and the presence of drusen (yellow deposits).
Geographic Atrophy (GA) – Advanced Dry AMD Geographic Atrophy is an advanced stage of dry AMD. In GA, there is a progressive loss of the cells in the macula, including the retinal pigment epithelium and photoreceptors, leading to well-defined areas of atrophy. This results in gradual, irreversible loss of central vision. GA can significantly impact daily activities such as reading, recognizing faces, and driving.
- Wet (Neovascular) AMD:
Less common but more severe, this form involves abnormal blood vessel growth under the retina, which can leak fluid or blood and cause rapid vision loss.
Symptoms
Patients with macular degeneration may experience:
- Blurred or distorted central vision
- Difficulty reading or recognizing faces
- Dark or empty areas in the center of vision
- Colors appearing less vivid
Peripheral (side) vision usually remains unaffected.
Diagnosis
Diagnosis of AMD and GA involves a comprehensive eye examination, which may include:
- Visual acuity testing
- Dilated fundus examination
- Optical coherence tomography (OCT)
- Fundus photography
- Fundus autofluorescence (especially useful for GA)
- Fluorescein angiography (for wet AMD)
Treatment Options
While there is currently no cure for macular degeneration, several treatments can help slow progression and manage symptoms:
- Dry AMD (including Geographic Atrophy):
- Nutritional supplements (AREDS2 formula): May help slow progression incertain patients with intermediate AMD.
- Lifestyle modifications: Smoking cessation, a healthy diet rich in leafy greens and fish, and UV protection are recommended.
- Geographic Atrophy Treatments:
- Recently, intravitreal injections of complement inhibitors (such as
pegcetacoplan and avacincaptad pegol) have been approved to slow the progression of GA. These treatments do not restore lost vision but can help slow the enlargement of atrophic areas.
- Low vision aids and rehabilitation can help patients maximize remaining vision and maintain independence.
- Wet AMD:
- Intravitreal injections of anti-VEGF medications (e.g., ranibizumab, aflibercept, bevacizumab) to reduce abnormal blood vessel growth and leakage.
- Laser therapy or photodynamic therapy in select cases.
Ongoing Care
Early detection and regular follow-up are essential for preserving vision. Patients are encouraged to monitor their vision at home using an Amsler grid and to report any sudden changes in vision promptly.
